I saw a MNK using this in similar fashion to Fell Cleave in Abyssea. How effective is this and what is the best setup?

It is very effective. I would say a warrior/sam would be more effective because of retaliation, but either is good with proper gear/atmas. Reason I would do cata on war is so you don't get any ruby lights and does 1700-2.7k dmg depending on mob and level/etc.
