Create your character, choose whether you want to start in Kanto or Hoenn, then the game proceeds mostly as normal. You can travel between the regions at some point later on too.
And NPCs aren't entirely HELP I AM TRAPPED IN 2006 PLEASE SEND A TIME MACHINE during battle anymore either; they can swap Pokemon around if they're at a disadvantage and such.
