You use glamour prisms to change the look
You have grade 1-5 and which you have to use depends on the item level of the item you want to show. If it's ilvl 61-80 then you need a grade 4, 81+ is grade 5 etc and each craft can make grade 1-5. Which you need depends on which craft is required to repair the item, you might need Grade 5 (Armorcraft) for example.
You then select glamour and the item you want and then it changes that item's look into the new item until you dispel it or glamour over it.

You can't glamour gear above your current level, gear from another job or gear that takes up more slots than the item you're wearing. If you want to glamour an item that takes up 1 slot but you're wearing a 2 slot (head+body) item then it won't work.
There are some exceptions though but that's because the armor displays over another piece while still taking up 1 slot.
