If my memory serves, this hat is obtained by finishing your server's final ballista battle royale round, it is awarded before heading off to the cross server finals. It must be quite rare as i have only seen one in the 3 years i've played ffxi. its teleport function sends you to Ru'lude Garden's at the cost of 50 Ballista points, and has Infinite uses.
