for me it is pvp. The poor beetle models front-right foot. It's been backwards for 14 years!
Various graphic issues, pertaining to stuff like the banners in Bastok having seizures, mobs disappearing if your camera is angled a certain way, seams in the terrain, visible borders and stuff around things like the bazaar icon and compass...
Removal of level restriction on quests like the Gustaberg Tour and Eco-warrior. The floating flower in Konschtat Highlands.

how the game disconnects if you sit too long at the yes/no prompt right before character selection
Do xbox live achiemvents transfer to xbox one somehow? Those would be missable forever!

The one thing I've wished for years is that you don't lose your TP if you WS out of range from the mob when it's moving away from you....this still annoys me

The way mobs circle around you till they are not in swing range if you mass pull. And if you try to adjust at all they continue to move. It's made me rage a few times.

the clipping delay when walking into/through people
Harden Shell animation bugged me for the longest time, glad they finally fixed it.
First thing that comes to mind is the water in Bastok Markets looks really graphically embarrassing when it spazzes out like this, wish they'd do something about it...
